Lance Alexander Polk, 18, of Fort Smith, was born March 26, 1989 and died February 17, 2008 in Fort Smith. He was a 2007 Graduate of Southside High School.
He is survived by his mother Chauntel Michelle Polk of Fort Smith; his father Chad Gaither of Heber Springs, AR; grandmother, Ann Griffin of Fort Smith and grandfather, C. Michael Gibson of Knoxville, TN; and his God Father Ronald Sitton of North Little Rock, AR.
Funeral service will be Friday, February 22, 2008 at 10:00 AM in the Edwards Funeral Home Chapel and cremation will be under the direction of Edwards Funeral Home following the service.
